Appointments must be scheduled to register.
To register, students will need the following records: birth certificate; medical records showing proper immunization and proof of physical (grade nine completed in grade six, grades 10 to 12 completed in grade 10); educational records from prior school and a copy of latest report card; two emergency telephone numbers and names; two proofs of residency in Naugatuck.
One proof of residency must be either a copy of a notarized lease or notarized statement from the property owner stating that the parent or guardian and child reside at the state address or a copy of a deed or mortgage or rental agreement.
The second proof can be a current utility bill or real estate or personnel property tax bill.
