EMS committee finalizes form

NAUGATUCK — The EMS Oversight Committee has launched a new way to track how satisfied borough residents are with their ambulance company.

Deputy Mayor and Chairwoman of the oversight committee Tamath Rossi announced the committee has finalized the complaint and commendation form for residents to fill out about Naugatuck Ambulance.

The form are available on the borough’s website, www.naugatuck-ct.gov, or the Mayor’s Office.

Residents who download the form from the borough’s website must fill it out, sign it, and drop it off or mail it to Naugatuck EMS Oversight Committee in care of the Naugatuck Mayor’s Office, fourth floor, Naugatuck Town Hall, 229 Church Street, Naugatuck, CT 06770.

“We will not accept emailed compliment and complaint forms. The reason being that we need to be very cautious about HIPPA,” Rossi said.

She explained that when a form is handed in, if there is any concern that it might contain personal information, the committee will contact borough attorney Ned Fitzpatrick.

The oversight committee had discussed the form during its meeting in October and November, and finally finalized it in December.

During the oversight committee’s November meeting, it heard input from Larry Santoro, president of Naugatuck Ambulance about how what information would be useful of a form.
