NEW HAVEN — Elizabeth A. Slivonik Kotak, 91, beloved wife of the late Andrew Kotak, died peacefully on March 9, 2015 at The Mary Wade Home of New Haven. She was born March 1, 1924 in Patchogue, Long Island, N.Y., daughter of the late Walter and Bronislawa Slivonik.

Mrs. Kotak was a long-time communicant of St. Joseph Church of Ansonia, taught knitting at adult education classes and was a member of the Windy Hill Girls Club, PTA, and Mother’s Group.

She is survived by her devoted children, Kathleen Harris of Bethany, Andrew Kotak, Jr. of New Jersey, Mark (Mary) Kotak of Berlin, and Rebecca (John) Devito of Massachusetts; cherished grandchildren, Franklin B. Harris of Michigan, Andrew DeVito of Stamford, and Adam Devito of Massachusetts; and niece, Carol (George) DePalma of Oxford. In addition to her husband, she was predeceased by a brother, Wesley Truskolaski.

A Mass of Christian Burial, meeting directly at church, will be celebrated on Friday, March 13, 2015, at 10 a.m. in St. Joseph Church, 32 Jewett St., Ansonia. Springtime interment will be in Three Saints Cemetery, Derby. The family wishes to express their sincere appreciation to the physicians and staff of The Mary Wade Home for their compassionate care in their mother’s time of need. The Wakelee Memorial Funeral Home, 167 Wakelee Ave., Ansonia, is in care of arrangements.
